Donor Advised Fund (DAF) Grant Request
For individuals who have setup their own DAF, please find a downloadable, fillable form below which needs to be submitted to Legacy Endowment by completing, printing out and signing, and then scanning and emailing or mailing to the contact information listed at the top of the form. It will take ten (10) business days to complete our review and if approved, the grant will be submitted shortly to the organization you have recommended. Instructions are also included below the form for signing electronically and saving the form. Then you can email us the form at [email protected].
Requirements to submit a DAF grant request
Request is for a minimum of $250.00 and above
Registered as a non-profit on the irs.gov site with a 900 or 990EZ submission within past two years
Grant recommendation does not support fraternal or political purposed organization
The grant does not underwrite the cost of advertising, sponsorships, tickets, galas, auctions, auction items, or fundraising events that directly benefits the Requestor.
The grant is not intended to cover membership costs unless the receiving organization can confirm that its membership fee is 100% tax deductible
The grant is not to be used the benefit any family member of the Requestor and is not earmarked to benefit any specifically designated individual
The receiving organizations is not a private, non-operating foundation
